  • What is your COVID-19 policy?

    We are all adapting during the COVID-19 pandemic, but that does not mean that your body has to miss out on physical therapy. Widespread concern persists in our community about the Coronavirus Disease 2019 (COVID-19), and we want you to know that your health is our top priority at Onofray Physical Therapy. Let's face it, the time we are spending social distancing is still valuable time to be focusing on your health and physical development.  


    While we're still offering in person therapy, physical therapy in Paradise Valley, AZ and nearby areas is also available for telehealth sessions at your convenience using our HIPPA compliant practice management software. This is available to all new and existing patients.


    The following precautionary measures have been implemented for in home physical therapy in Phoenix, AZ and surrounding areas: therapist personal health screening with daily temperature check, diligent hand hygiene (sanitizing and/or using soap and water on entering and leaving the home), wearing a mask and gloves, and bringing minimal equipment into or out of the home. All individuals in the home should be fever and symptom-free for at least 48 hours. Also, we will not be treating you if your therapist or any of her family members have any symptoms or fever.

  • What are your Specialties?

  • Do I need a doctor's prescription?

    No, you do not need a doctor's prescription. Per Arizona State Law, a person may see a physical therapist via direct access. In other words, you have the ability to be evaluated and treated by a physical therapist without being referred by a doctor or other healthcare practitioner. 


    However, if you have a prescription, we will gladly accept it and correspond with your physician as needed while we provide the physical therapy Phoenix, AZ trusts.

  • Are your services covered by insurance?

    We have out-of-network status with most private insurance companies and plans. We do not bill any insurance companies for physical therapy services. Payment in full is expected at the time of service. Upon request, an itemized bill (Superbill) can be produced for a client and the client is responsible for submitting this bill for reimbursement or credit towards their deductible. Although most insurance companies and plans offer some type of credit or reimbursement for our services, this is not guaranteed and it is always the responsibility of the client to verify his/her benefits. Onofray Physical Therapy is a cash-based, mobile physical therapy practice. We feel strongly about providing you with the best physical therapy in Paradise Valley, AZ, which you deserve and need. We believe you should not be limited to a set number of visits or confusing billing codes. Your body tells us how many visits you need and what services work for you. Payment can be made via cash, check, major credit card, FSA/HSA bank card, and select online bank transfer services (Zelle and Venmo).

  • How much does a session cost?

    Onofray Physical Therapy offers a unique, one-on-one, hands-on physical therapy experience. 100% of your time is spent with a physical therapist at the location of your choice. Whether you need in home physical therapy in Paradise Valley, AZ or want to utilize our telehealth services, we provide all-inclusive pricing. There are no extra costs for an evaluation and no hidden fees for additional treatment techniques. 


    We offer several payment structures that are tailored to meet the needs of each individual client. Onofray Physical Therapy has payment options with rates that are extremely competitive with traditional out-client therapy clinics and offer many additional conveniences and benefits.

  • Can I pay with HSA or FSA account?

    Yes, you can pay with the funds in your HSA or FSA account. While our preferred methods of payment are cash, check, Zelle, or Venmo, we also accept debit cards, HSA/FSA cards, and all major credit cards.   • Where can I be seen?

    Onofray Physical Therapy is based in the North Phoenix area and serves a large part of the North Valley area including Phoenix, Peoria, Glendale, Anthem, New River, Cave Creek, Carefree, and Scottsdale.


    With a mobile practice, we have the freedom to see you wherever is most convenient for you! We can meet in your office, gym, pool, favorite park, or even at your house for in home physical therapy Phoenix, AZ and the North Valley area trust.   • What if I do not want to be seen at home?

    That's not a problem at all. We will meet you almost anywhere that is convenient for you. That could be your office, gym, church, or practically anywhere else. The possibilities are endless when you choose the provider of physical therapy Phoenix, AZ loves.

  • How can Onofray Physical Therapy take place at my home? What about all the equipment needed?

    Onofray Physical Therapy has everything for you to have an unmatched physical therapy experience. We bring all the tools we will need, including the treatment table, soft tissue mobilization equipment (cupping tools, massage location), dry needling supplies, weights/resistance bands, taping supplies, and much more! Whether you're looking to treat your shoulder pain in Scottsdale, AZ or to live a healthier life in Phoenix, we have the tools to help you.

  • What can I expect when you visit?

    On your first visit, you can expect to be interviewed and evaluated to find the root cause of your symptoms. Objective measurements such as range of motion (ROM), strength, flexibility and functionality will be assessed. Your therapist will provide a detailed assessment. Your goals will be identified, and a comprehensive treatment plan will be established.  Treatment will include a one-on-one, hands-on approach to meet your goals. Most of the session will focus on manual techniques however after care exercises (homework) will be provided as well.  You can schedule visits for 60 or 90 minutes. 

  • What types of injuries or conditions are NOT treated?

    Onofray Physical Therapy specializes in orthopedics and sports medicine. We have expertise in the care of most musculoskeletal conditions. We do not specialize in many neurologic or vestibular conditions.  However, we would be happy to refer you to another provider if your condition or situation is not a good fit. 

    Some examples of pathologies that we do not treat:

    • Post- concussion Syndrome 
    • Vertigo/vestibular dysfunction
    • CVA/Stroke
    • Parkinson’s
    • Post- amputation 
    • Pelvic Floor Dysfunction

  • What is your cancellation or appointment policy?

    At Onofray Physical Therapy, we are passionate about goal setting. We know that your recovery will be most successful when we work together. Our unique and individualized service is designed to optimize your success while minimizing setbacks and regressions. Keeping all your scheduled appointments is critical to your overall recovery, but also, the speed with which you will be able to return to your activities and passions. We expect all patients to provide us with a minimum of 24 hours’ notice to cancel their appointment. Unforeseen events do happen; one late cancellation is allowed during your episode of care. Future late cancellations will incur charges payable on the day of your next treatment.   • Why should I see you instead of going to another Physical Therapist?

    The standard visit with a physical therapy provider typically consists of 15 minutes of hands-on care by the therapist.  The rest of the session is filled with exercises supervised by a technician or assistant. 


    Onofray Physical Therapy provides the undivided attention of our therapist for the full session. Our techniques include hands-on manipulation, Cupping, Dry Needling (if requested/required) and exercises tailored to your specific needs.


    In the current American payer-driven system, insurance companies often dictate everything about the course of your care in a traditional physical therapy clinic. An insurance company employee, who is incentivized to reduce health care expenditures, should not be the party that decides: how long you can be cared for, how often you can be cared for, how many times in a year you can be treated, or what interventions you will be allowed to receive. That same insurance employees often decides whether your treatment is “medically necessary” and where you can be treated. YOU, as an individual and health care consumer, has the RIGHT and the OPPORTUNITY to choose to become part of an innovative model that prioritizes YOUR health and allows YOU to be seen by a musculoskeletal expert that gives you the best chance to succeed and achieve the physical freedom you have been hoping for. 

  • Do you offer virtual appointments?

    Telehealth visits are an option for your care as it is our priority to keep you feeling safe and comfortable. Zoom meetings can be utilized to evaluate your situation and develop an effective treatment plan.
